| JOB SUMMARY: All employees are expected to perform their duties in alignment with the vision and values of the organization. A social worker helps people achieve more adequate, satisfying and productive social adjustments through the application of social work principles and methods, which includes, but is not limited to: DUTIES AND RESPONSIBILITIES: (List essential functions in order of responsibility) 1. Age-appropriate, psychosocial evaluation and treatment: newborn, child, and adolescents, adult, elderly. 2. Counsels using applied psychotherapy of a non-medical nature with individuals, families, or groups. 3. Provides information and referral services; 4. Provides or arranging for provision of social services; 5. Explains or interpreting the psychosocial aspects in the situations of individuals, families, or groups; 6. Helps communities: to organize, to provide, or to improve social or health services 7. Assumes the discharge planning functions for complicated cases that require greater than 30 minutes of time for completion. 8. Performs other duties as assigned. Attends and participates in team meetings/rounds or assigned committees 3. Participates in Community Outreach activities 4. Assist with training staff, interns and volunteers 5. Participates in Performance Improvement Activities, as assigned 6. Maintain department specific patient records: face sheet and productivity information 7. Completes and submit mandated reporting documentation within 24-48 hrs. 8. Provides appropriate information and referral 9. Provides consultation to staff and outside agencies 10. Documentation in patient’s medical records (filed within 24 hours) 11. Formulate & implement age-appropriate, population specific, culturally, spiritually sensitive, individualized care plan 12. EDUCATION, EXPERIENCE, TRAINING: (include Licenses and Professional Affiliations) Experience: Minimum 1-year experience in health/medical setting Education: Master’s degree in Social Work/Welfare required.
